A consultation can leave you with a mixture of explanations, estimates and questions. Keeping a short written summary helps you separate what was actually proposed from what you still need to clarify. Use the template below during or immediately after the conversation, then ask the clinician to correct any misunderstanding.
This is a personal note-taking aid. It is not a medical record, a treatment recommendation or a substitute for the clinic’s written plan. You do not need to fill every box with an answer before leaving: an explicit “not yet discussed” is more useful than an assumption.

| Written documents | Document name, date and version received | ________________ |
| Open questions | What still needs explanation or examination | ________________ |
| Next step | Who will do what, and when to expect an answer | ________________ |

Keep estimates separate from decisions
When recording a number, include its unit and the conditions attached to it. Do not shorten “preliminary estimate, subject to examination” to a definite promise. If you are unsure whether a figure refers to grafts or individual hairs, leave a question beside it and ask for clarification. Our guide to grafts and hairs explains the terminology to discuss.
Likewise, record a quotation as a quotation. Note its date, currency, listed inclusions and exclusions. A package description does not by itself establish the details of a clinical plan. Ask for the relevant written documents rather than treating a handwritten summary as the final agreement.
Use three simple labels
Mark each note as “explained,” “preliminary” or “unanswered.” These labels describe the status of the conversation, not the quality of the treatment. They make the next exchange more focused: instead of asking the clinic to repeat everything, you can identify the exact point that remains unclear.
For example: “Top-of-scalp coverage discussed; final area not yet confirmed” is clearer than “full coverage agreed.” Write down what you heard, without filling gaps from a promotional photograph, another patient’s experience or an online estimate.
Ask the clinician to check your understanding
Before ending the appointment, read back the main proposal and the open questions in your own words. Ask which statements need correction and which require more assessment. The NHS consent guidance describes the importance of information about treatment, benefits, risks and alternatives. We use those subjects to organize this template; UK guidance does not establish the consent rules in Armenia.
Store the summary with the documents
Keep the original proposal and any later version together with the date of each discussion. Add corrections as dated notes instead of silently replacing earlier information. If you consult more than one clinic, use a separate sheet for each so that names, estimates and instructions do not become mixed.
For international care, ask separately about medical records and follow-up arrangements. The CDC recommends planning for continuity of care and obtaining relevant records. Your personal notes help organize questions but do not replace those records.
To clarify an item on your summary, contact United Hair Clinic and identify the specific question. Avoid sending unrelated personal documents by default; ask how requested information should be provided.
